To request an accident report occurring within the boundaries of Washington County, please fill out the form below.  Please allow up to 5 business days for processing of your accident report.

Accident reports can also be requested via USPS by mailing to:
Washington County Sheriff’s Office, 444 South 16^th St., Blair, Nebraska 68008 or calling the Business Office at 402- 426 -6873

Request Process:

1. Enter the necessary information into the form below. All required (*) fields must be completed.
2. Select the delivery method desired: email, fax or United States Postal Services.
3. Your accident report will arrive via your selected delivery method.
